Change of Major
Prior to completing the change of major form, please schedule a meeting via STAR Balance with your academic counselor to discuss changing your major.
Deadline: Last day for a 100% refund.

Health Immunization Clearance Form (PDF)

Placement Test Self-Reporting Form
Self-Report your SAT, ACT, Smarter Balanced Assessment, HiSET, GED, 12th grade English/Math course grade, and/or Cumulative GPA to see if we can place you in the appropriate English and math classes. VA students need to submit official test scores.
Transcript Request
Request an official copy of your Honolulu Community College transcript to be sent electronically or via postal mail

Application Rollover Request Form
Request to rollover your application from a previous term, if HonCC was your last home campus and your last record was not inactivated.

Catalog Year Change Form
Use this form to change the catalog year for which the curriculum requirements are in effect for your current major.
Change of Address and/or Phone Number
Request to change your permanent address or phone number
Consent to Release Confidential Information
Request to share information from your student record to another party. This form cannot be used to release health clearance and financial aid information.
Course Add Form
Request to add a class to your schedule after the last day to add a class as posted in the academic calendar, take a course without having taken the prerequisite course, take the course without enrolling in the corequisite course, or take the course outside of their declared program of study. If you are adding a class on or after the first day of instruction, you may be assessed a $30 late registration fee.
